Onglyza (saxagliptin) was approved by the U.S. Food and Drug Administration (FDA) in 2009 and Kombiglyze XR (saxagliptin and metformin hydrochloride extended-release) was approved the following year. The two medications are in a class of drugs called dipeptidyl peptidase-4 (DPP-4) inhibitors which inhibits an enzyme in the body that reduces insulin production. In April 2015, an FDA committee voted in favor of the FDA requiring saxagliptin to carry a warning label about the risk of heart failure after reviewing a study that found a 27 percent increase in the hospitalization rate among patients who were prescribed the drug. One committee member voted to take the drug off the market. The study connecting saxagliptin with heart failure was published in The New England Journal of Medicine in October 2013.

The FDA has been investigating Onglyza and Kombiglyze XR since 2013 after the federal agency’s adverse events database showed patients had experienced additional side effects allegedly from taking the drugs. From 2004 to 2009, the FDA received reports of pancreatitis or inflammation of the pancreas, pancreatic cancer and thyroid cancer. By August 2015, the FDA found that severe and disabling joint pain had been associated with DDP-4 inhibitors. According to a review of adverse events reports submitted to the FDA, the pain symptoms started from 1 day to years after they started taking a DPP-4 inhibitor. The pain symptoms went away after patients stopped taking the medication. The federal agency added a warning and precaution about the joint pain risk to the labels of Onglyza and Kombiglyze XR.
