As Onglyza heart failure lawsuit claims raise concerns over the diabetes drug’s link to medical conditions including pancreatic cancer, a study has associated Onglyza with the potential to cause heart failure in patients who are prescribed the drug to treat type 2 diabetes. A randomized, double-blind, placebo-controlled clinical trial, called SAVOR, was conducted with 16,492 patients with type 2 diabetes and cardiovascular disease or risks of cardiovascular disease. Some patients were prescribed saxagliptin (the generic name for Onglyza) while others were given a placebo. The patients were followed for a little over two years. Researchers found that patients who took saxagliptin had a 27 percent higher risk of hospitalization for heart failure than patients who took a placebo. Patients who took saxagliptin also had a significantly increased risk of “all-cause mortality,” or overall death, than those who were not prescribed the drug. The SAVOR study was published in the New England Journal of Medicine in 2013. After reviewing SAVOR ‘s findings, the FDA asked AstraZeneca to investigate the potential link between Onglyza and heart failure.

The U.S. Food and Drug Administration (FDA) approved Onglyza in 2009 and Kombiglyze XR in 2010 as drugs that help lower blood sugar in adults with type 2 diabetes by helping the body increase the level of insulin after meals. When left untreated, type 2 diabetes can lead to serious problems, including blindness, nerve and kidney damage, and heart disease. Onglyza and Kombiglyze XR are in a class of drugs called dipeptidyl peptidase-4 (DPP-4) inhibitors, which inhibits an enzyme in the body that reduces insulin production. DDP-4 inhibitors, however, have side effects. In August 2015, the FDA warned that saxagliptin and other DDP-4 inhibitors may cause severe and disabling joint pain. The warning and precaution about this risk was added to the labels of all drugs in this class.

In addition to reports of heart failure, the FDA began investigating reports in March 2013 of Onglyza and other incretin mimetic drugs prescribed for type 2 diabetes. Incretin mimetic drugs work by mimicking the incretin hormones that the body usually produces naturally to stimulate the release of insulin after a meal. According to the FDA, a review of the agency’s adverse events database from 2004-2009 showed patients who were prescribed the drugs to treat type 2 diabetes reported pancreatitis, pancreatic cancer, and thyroid cancer.
